*Every participant must sign this waiver before boarding the vessel.

1. ASSUMPTION OF RISK

I, the undersigned, understand that
participating in a fishing charter or eco-tour involves inherent risks, including but not limited to:

    •    Slippery decks, trip hazards, and potential falls

    •    Sea sickness, sun exposure, and unpredictable weather conditions

    •    Risks associated with fishing equipment, hooks, and marine life

    •    The possibility of injury or death from boating accidents, drowning, or encounters with wildlife

I acknowledge that Captain Grow
Fishing Charters, its owners, employees, and affiliates have taken reasonable
measures to ensure my safety, but they cannot eliminate all risks associated
with these activities.

2. RELEASE OF LIABILITY &
WAIVER OF CLAIMS

In consideration of being allowed
to participate in a charter trip or eco-tour, I voluntarily agree to the following:

    •    I assume full responsibility for any risk of injury, property damage, or death
while participating in any activities related to Captain Grow Fishing Charters.

    •    I release, discharge, and hold harmless Captain Grow Fishing Charters, its owners, captains, crew, and affiliates from any liability, claims, demands, or
legal actions arising out of injury, damage, or loss, whether caused by
negligence or otherwise.

• I understand that I am responsible for my personal belongings, and Captain Grow Fishing Charters is not liable for lost or damaged items.

    •    I confirm that I am physically able to participate in the activities and do not
have any medical conditions that could be aggravated by this experience.

3. SAFETY & CONDUCT AGREEMENT

I agree to:

    •    Follow all instructions given by the captain and crew.

    •    Wear a U.S. Coast Guard-approved life jacket when instructed.

    •    Refrain from reckless or dangerous behavior that could endanger myself, other passengers, or the vessel.

    •    Not bring prohibited items such as illegal drugs, excessive alcohol, or weapons on board.

    •    Understand that the captain has the final authority on all matters related to safety and trip operations.

4. MEDICAL EMERGENCY AUTHORIZATION

In the event of a medical
emergency, I authorize Captain Grow Fishing Charters to administer first aid
and, if necessary, arrange transportation for emergency medical treatment. I
understand that I am financially responsible for any medical care required.

5. PHOTO & VIDEO RELEASE
(OPTIONAL)

I  ☐ DO,  ☐  DO NOT authorize Captain Grow Fishing Charters to use photos or videos taken during my trip for promotional or marketing purposes.

 6. MINOR PARTICIPANTS (UNDER 18)

A parent or legal guardian must
sign for any participant under the age of 18. I certify that I am the
parent/guardian of the minor(s) listed below and consent to their
participation.
